Care Partners: Palliative Care, End-of-Life, and Caregiver Support
Care Partners provides supportive care on the journey of aging and serious illness. Our RN Care Coordinator can help you navigate health care decisions, the complexities of living at home with a serious illness, or the feelings and tasks of end-of-life. She also provides coaching for caregivers and can connect you with many forms of support. Care Partner volunteers provide companionship in the home or a quiet presence at end-of-life. What is Palliative Care?
Palliative Care is treatment that enhances comfort and improves the quality of life of an individual during and through debilitating chronic or life threatening illness. The goal is to relieve suffering through the prevention or alleviation of physical, psychological, social and spiritual distress. Palliative care is patient and family centered. It begins at the diagnosis of a life threatening illness and continues through cure or until death and into family's bereavement period.
